Gemming

On most hard modes the main tank, even in the best gear, will be hit for extreme amounts of damage in one hit. The only effective way to handle these hits is with Holy Light. This would not be a problem with any gem set if the fights were under 5 minutes. Many are not, however, and pure intellect gems reign supreme in hard fights.

Getting Socket Bonuses

You'll want to use the Insightful Earthsiege Diamond meta gem and use Nightmare Tear to cover the gems to get this, since you'll use pure Brilliant King's Amber everywhere.

You can easily ignore socket bonuses on the rest of your gear and gem pure Brilliant King's Amber gems in all sockets.

Enchanting

Enchanting is fairly simple. You want to maximize spellpower,mp5, haste, crit, and intellect. To do this, the following enchants should be used.

Helm:
Arcanum of Blissful Mending
Wyrmrest Accord - Exalted

Shoulders:
Greater Inscription of the Crag
Sons of Hodir - Exalted

Back:
Enchant Cloak - Greater Speed
Enchanting 430

Chest:
Enchant Chest - Exceptional Mana

The exceptional mana provides more mana, even with kings. Plus, it's way cheaper.

Wrist:
Enchant Bracers - Exceptional Intellect

This is a bit controversial, but the intellect enchant gives 276 mana to a holy paladin instead of the 30 spellpower alternative. It is sort of personal preference, but stacking enough intellect can min/max your mana pool and efficiency.

This is also a bit hard to decide. If you pick up Pursuit of Justice in your spec you should definitely get Icewalker. With a traditional 51/20/0 spec, however, you should use Tuskarr's.

Weapon:

These two enchants are both very good. Mighty Intellect provides 517 mana to a holy paladin without kings, as well as 0.24% crit and 7 spellpower. Again, if you're min/maxing then intellect is the better enchant. A holy paladin with high levels of gear can have enough spellpower.
